Help with PDF Documents
*The online form works best using desktop computer with Firefox (Click to download FREE app). Any other browser should work but using <TAB> may take you in a strange order through the fields. Using the mouse to click each field will allow you to enter info then print.
*If you need a PDF Reader application use Acrobat Reader DC (Click to download FREE app) . *Uncheck McAfee and Chrome offers
*On Cellphone: Ability to download / print from smartphone will be limited to user’s experience performing this type of action.
